Westside Family Healthcare is a nonprofit organization committed to providing high-quality primary medical care without regard to the ability to pay. As an Ambulatory Care Registered Nurse (RN), you will play a vital role in our patient care team, working directly with high and rising risk patients to coordinate their care throughout the continuum.

This position is designed for dedicated RNs who are passionate about supporting patients through education on disease processes, self-management skills, and facilitating the implementation of prescribed medical treatment plans.

KEY RESPONSIBILITIES:

1. Manage a caseload of patients at risk for health deterioration by utilizing the nursing process and established protocols. Collaborate with patients, families, and the care team to implement effective care plans.
2. Engage in daily huddles with the Clinical Operations team and participate in scheduled team meetings and trainings.
3. Continuously review patient medical records to identify care needs and provide timely interventions.
4. Work with patients and families to address both medical and psychosocial needs, identifying barriers to adherence to care plans.
5. Educate patients and families about medical conditions and self-management techniques.
6. Incorporate lifestyle improvement opportunities into patient interactions to empower health goal achievement.
7. Assess and document patient progress, utilizing clinical judgment and established protocols.
8. Facilitate access to appropriate care and promote preventive health behaviors.
9. Provide ambulatory care support, which may include walk-in or telephone triage using established protocols.
10. Advocate for patient care issues to ensure quality and alignment with the care plan.
11. Participate in quality improvement initiatives and complete other assigned duties.
12. Maintain compliance with professional and organizational standards.

Minimum qualifications include: graduation from an accredited nursing program with an associate degree, active RN licensure in Delaware (or compact state) with no restrictions, current BLS certification, and commitment to obtaining Ambulatory Care Certification within one year of hire.
